    Description: The purpose of this Notice of Funding Opportunity (NOFO) is to develop and implement multi-faceted strategies that support the delivery and sustainability of evidence-based chronic disease self-management education (CDSME) programs for older adults with one or more behavioral health conditions.The two primary goals are: Goal 1: Develop capacity to increase the number of older adults with one or more behavioral health conditions who participate in evidence-based self-management education and support programs.Goal 2: Develop and disseminate resources that will enhance the participation of older adults with behavioral health conditions in evidence-based self-management education and support programs. All resources must be 508 compliant.Successful applicants will also be expected to (a) Develop a range of cross-sector partnerships to achieve project goals and objectives; (b) Develop and/or implement appropriate trainings and tools for aging and behavioral health professionals; and (c) Develop a toolkit detailing successful (and unsuccessful) strategies, lessons learned, and recommendations that will serve as a guide for replication of efforts.The Administration on Aging (AoA), part of the Administration for Community Living (ACL), plans to award 3 cooperative agreements with a three-year project period, subject to availability of funds.
