Additional Information on Eligibility: If the applicant is other than a Department/Ministry of Health in the specified jurisdiction, the applicant must submit a memorandum of agreement/memorandum of understanding with the Department/Ministry of Health in the jurisdiction for which it is applying. CDC will need certain surveillance and health system data from applicants in order to ensure performance measures are being met for the benefit of achieving public heath results and hence, applicants must demonstrate they have the authority to submit the data to CDC and will have access to the data. The MOU/MOA should specify that the Department/Ministry of Health will provide the applicant access to surveillance and health system data, has granted the applicant the authority to submit the data, and will collaborate with the applicant on this NOFO to ensure complete data are submitted to CDC according to established guidelines and processes. The MOU/MOA must be provided in an attachment labeled “MOU/MOA”; when submitting via www.grants.gov. Foreign Institutions, Non-domestic (non-U.S.) components of U.S. Organizations are not eligible to apply. Non-domestic (non-U.S.) Entities (Foreign Institutions) are not eligible to apply. CDC will consider any application requesting an award higher than the ceiling amount of $1,400,000 to be non-responsive. Applicants must provide proof of ability to serve populations in one of the jurisdictions listed below: Commonwealth of the Northern Mariana Islands (CNMI), American Samoa, Guam, Federated States of Micronesia (FSM), Republic of the Marshall Islands (RMI), Republic of Palau, U.S. Virgin Islands (USVI), Puerto Rico, Evidence of ability may include documentation that the applicant is a territorial government or a bona fide agent in the CNMI, American Samoa, Guam, the FSM, the RMI, the Republic of Palau, USVI, or Puerto Rico. Documentation must be provided in attachment labeled “Evidence of Ability”; when submitting via www.grants.gov. A bona fide agent is an agency/organization identified by the state as eligible to submit an application under the state eligibility in lieu of a state application. If applying as a bona fide agent of a state or local government, a legal, binding agreement from the state or local government as documentation of the status is required. Attach with “Other Attachment Forms”; when submitting via www.grants.gov.Applications that do not comply with these requirements will be determined non-responsive and will not receive further review.
Description: Six out of ten adults living in the United States have at least one chronic disease. The cost of managing individuals with chronic conditions is a significant portion of the nation’s healthcare expenses. The United States has made progress in chronic disease prevention and control over recent decades. However, similar advancements have not been made in the U.S. territories and freely associated states.The U.S. territories and freely associated states include:American SamoaThe Commonwealth of the Northern Mariana Islands (CNMI)The Federated States of Micronesia (FSM)GuamPuerto RicoThe Republic of the Marshall Islands (RMI)The Republic of PalauThe U.S. Virgin IslandsThe unique challenges these islands face make addressing chronic disease particularly complex. These challenges include:High disease burden.Dispersed populations.Limited infrastructure.Diverse cultures and languages.Vulnerability to natural disasters.Strained healthcare systems.High healthcare costs.This cooperative agreement will support integrated, evidence-based strategies and activities to prevent and manage chronic disease in the U.S. territories and freely associated states. This notice of funding opportunity (NOFO) aims to reduce disability and death rates associated with chronic diseases by decreasing the prevalence of modifiable risk factors that contribute to chronic diseases in these islands. Focus areas include: Preventing and reducing tobacco use and secondhand smoke exposure.Preventing and managing diabetes.Improving oral health disparities.There are two components to this NOFO. The first is a required Core Component, which uses evidence-based strategies to promote health and reduce chronic disease. The second is an optional, competitive Oral Health Component, which addresses oral disease with evidence-based interventions and practices.
